They also share ways to prevent HIV and/or reduce harm, facilitate education groups, complete HIV screenings in cooperation with the Public Health Department, lead pre-test counseling, make referrals for other needed services, and provide follow-up on clients. Further HIV services include intensive case management to HIV positive clients who are using drugs/alcohol. Services include a thorough assessment, an individualized action plan that includes entry into substance abuse treatment if needed, medical case management services from a nurse, medication management, assistance with transportation, weekly specialized counseling groups, and others.
