Supported Home Living
Supported Home Living program offers support to eligible persons living in the community.
Trained and experienced staff provide persons with daily living skills and other
individualized services as determined in their “Person Directed Plan.”
Services may be paid through Medicaid Home & Community-Based Services
(HCS), Texas Home Living (TxHmL) or general revenue funds. Services Include:
Foster and Companion Care
- Foster Care and Companion Care (HCS Only) provides care to one to three persons
with disabilities in an apartment or home shared by both parties. The provider may
or may not be related to the consumer. Both foster and companion care help the consumer
with daily living activities such as grooming, eating, dressing, and personal hygiene—as
well as independent living task such as securing and providing transportation, housekeeping,
etc.
- Community Supports Staff provides the person with assistance in managing
their finances, transportation, grocery shopping, housekeeping, and accessing community
activities and services.
- Respite Care Staff provides supervised emergency or short-term care. MR Services
offers in-home and out-of-home respite. MHMR of Tarrant County operates one out-of
–home overnight facility. Additional respite is offered through contracted providers.
Eligibility

Must live in Tarrant County

Must be age 21 years or older

Must have a diagnosis of mental retardation, autism, or pervasive developmental
disorder
